Taking A Baby Overseas Needs An Infant Pasport
The law mandates that all international travel be accompanied with a valid passport. There are no exceptions. Newborns to age 16 must acquire a minor or infant passport by applying in person with parents in attendance. It is the responsibility of the parents to complete the application forms and bring the necessary, designated documents to the acceptance office.
Form DS-11
Form DS-11 is the application submittal applicable to all minors. This form requires identification information and future travel plans. The U. S. Department of State website allows parents to fill out Form DS-11 electronically and print a copy for submission on infants. This form can be acquired at any acceptance facility. Handwritten submissions require legibility in black ink. The applicant’s signature will be witnessed by a facility agent or, in the case of an infant, the agent will instruct guardian signage. Proof Of Citizenship
U. S. citizenship documentation on the minor is necessary when filing submission Form DS-11. Proof is satisfied through a valid birth or citizenship certificate or consular report of birth abroad. It is prudent to make a copy for safekeeping to avoid any remote risk of damage or loss. Your original paperwork will be returned along with your new legal travel document by mail.
Proof Of Legal Guardianship
Proof of legal guardianship is required on all minor children for international travel. An appropriate birth certificate, U. S. or foreign, or consular report of birth abroad that includes both parental names, satisfies evidence of relationship to the minor child. In the case of adoption, the decree granting such must include both adopting parties or a court order that grants custody or guardianship. When it is impossible for both parents to be in attendance, the absentee parent must submit a notarized Form DS-3053 Statement of Consent. Your documentation will be returned via mail.

With the submission of an application for a minor, guardianship must be proven with proper identification of the parents or legal custodians. This is accomplished with a valid passport, driver's license, social security card or military ID. See the U. S. Department of State for all acceptable forms of ID,which must be photocopied on both sides on standard, 8-1/2 x 11" white paper.
Photograph
Photographs are a requirement with each application. These two, identical, headshots of the minor child must be recent (no more than 6-months prior) and measure a size of 2" square per Department of State stipulations. Document-compliant photographs are available at many drugstores, camera shops and local AAA offices. Call ahead to insure the availability of suitable photography equipment for acquiring newborn, legally acceptable photos.
Fees
Processing and production of the legal document incurs a fee. The cost is determined upon the need for either the more extensive passport book or more limiting card. The book currently costs $85. The "book" is mandatory for all international visits by air travel. The lesser cost is for a "card" at $35. The smaller size of the card fits inside a wallet but carries restrictions as it only allows the child to travel by land or sea to Canada, Mexico and the Caribbean.
A passport puts the whole world in your hands. Allow a minimum of two-weeks to receive the legal document. An infant pasport has an expiration date of 5-years.
